Final Follow Up Requirement
We realize many projects take a significant amount of time to implement and evaluate so your Final Follow Up Requirement will be due within 18 months. The Foundation will outline specific follow-up requirements. You will be able to submit your follow-up information online using the account you created for the application. Your “account user name” email address will receive notification of accessibility to the requirement feature for each grant you might have received. You can sign on to your account, review the instructions there and submit the follow up report requirement on line.
